Outlier Health and Wellness is a House call and telehealth medical practice that provides competent medical care in the patient's homes as well as virtually. We specialize in providing primary care for the elderly and chronic care homebound patients. We provide transitional care visits for those patients recently discharged from a hospital or facility such as a rehab or nursing home. This service allows patients to have a medical provider care for them in their homes until they are stable enough to continue outpatient visits with their primary care provider. Virtual/telehealth visits are available for patients that are stable and need follow-up on a chronic condition. These visits are also used for simple problem visits and our medical marijuana visits. Our certified providers offer certifications and prescriptions for medical marijuana to qualified eligible patients living in New York State.
